I had a problem with my phone and now when I put my chip back in it tells me that I need a PUK. Where / how do I find this

log in to my o2 and it is in there

@MI5 thought that insecure method was going to be removed @MercedesS can you look into this anybody can use that form to unlock a sim locked handset by just knowing the mobile number

Certainly less secure than logging into MyO2 but not likely that any random person could find a sim card and know the number to enable it to be unlocked.
I wonder how many times that process has been used to fraudulently obtain a PUK?

Yes that's correct. As well as showing outdated phones etc. @Toby was dealing with it initially and passed it on to @Martin-O2. I gave Martin the relevant information on the 20th February when he contacted me.
