You normally have to ring O2 Customer services however your best bet would be to print out a claims form and email it across. form link is https://www.o2.co.uk/sites/default/files/2017-04/o2insurance_claimform.pdf

For a fault, you could get it repaired free under warranty.
Insurance carries an excess and may not cover faults either.
Contact the manufacturer for best results.
