Keeping old number - Incorrect security info

Hello. I just bought a sim only plan with O2 and my sim has arrived.

I was previously with 3 and I'd like to keep my number because many of my online accounts use two-factor authentication. 

 

The problem is when I submit a form for O2 to keep my number, I'm told my information is incorrect. 

 

 

My old number was registered by my mum as I was an early teen back then. I've been using and paying for that number for years now and have changed most of the details for my own, but I'm clueless as what to what info is wrong or what exactly the form wants from me.

The rejection email gives me no information on what is wrong, O2 have literally no way to contact them other than this forum. I'm left clueless. 

 

If someone can give me guidance I'd really appreciate it.
