If within cooling off period just return it, if it is of poor build quality etc why bother to unlock it?

Exactly @adamtemp64 what's the point in that. @Anonymous If you think the phone is that bad. Return it in the next 7 days to change your mind rather than unlock it then you would be stuck with a phone that you say is poor

I would recommend trying again as:
1. The process now is automated and requires no form
2. The wrong device may be registered in my o2 considering it was only released 5 days ago and I'm guessing you filled out form in first day (nearly a week?) and the imei may not have latched by time form filled in
