Why after numerous times contacting customer service to reactivate my old number, still a negative response, very frustrating. Maybe best seeking a new provider after being with them for years.

Once a number has been cancelled the chance of been able to recover it are next to zero, same if you port in.. 

There are lots of other (and cheaper options) so do some research and jump ship..  Details on how to cancel your contract her Guide: Cancelling Your Contract

Assume this is PAYG that has become deactivated. Depends on how long ago that happened. There is never a guarantee in such cases.

 

If it is another scenario what happened?

This is not O2 and we are all customers here similar to yourself and cannot answer account type queries.
