Check your settings against those listed here http://www.geeksquad.co.uk/articles/o2-apn-settings#Android
It may also be necessary to ask CS to reprovision MMS on your account.
Can I just check that you have a contract sim card too and not a PAYG one....?
Other ideas are to forget about sending MMS and use whatsapp, BBM, Facebook or email to send if possible.
I have no affiliation whatsoever with O2 or any subsidiary companies. Comments posted are entirely of my own opinion. This is not Customer Service so we are unable to help with account specific issues.

Currently using:
Pixel 7a (O2 & Lyca), One Plus 6 (Sfr), iPhone 12 Pro Max (Vodafone)

@MI5 is your man, he'll be along soon.
But what is suggest is getting a new sim and performing a sim swap at http://swapmysim.o2.co.uk
Could you also make sure on the note 4 you have mobile data activated.
