I'm presuming that volume is set to maimum on the handset?

 

Is there any cover/case etc. that might not have been machined properly blocking the ear piece hole(s)?

Have you tried checking the settings? Click volume button up/down on any page and to the right hand side there will be a settings option. Make sure everything is turned up.
