You have answered your own question without knowing it.
Why do they charge extra? You have ordered despite knowing that they do, so in essance the answer is 'Because they can'!
I was at the O2 store, ready to upgrade, when they released their prices this morning. I took one look at the total cost for two years and ordered from apple and immediately switched my tariff down to 2GB/Unlimited call/Texts for £15.30 a month.
So by O2 Adding £80 to the phone cost of the phone I was looking at, I will now be paying them £367.20 over the next two years instead of £1,295.99, saving myself £229.79! Way to lose money O2!

This has happened since the launch of refresh tariffs and will not change

the refresh tariff is both good and bad you can get some real bargains if you look closely especailly on refusrbished models while some are more expensive for example the new iphone 7. i do like the fact you can upgrade each year or whenever you want to as long as you pay the remainder of the balance. They do need to improve the offer with both EE and now Apple doing something similar with no added extras it won't be so enticing for new customers.

Well that's incorrect having been on Refresh tariffs since inception, you maybe find a £10/£20 disparity but never to the extent of the £80-£90 your seeing on the iPhone 7. Galaxy S7 edge at present on refresh costs £639.99, the same as Samsung direct. O2 have clearly decided to fleece the product this time, such a shame people didn't notice it before hand but that was my point earlier it's a blatant sneaky cost proving basic concepts of treating the customer fairly are not in O2s mission statement.

From this community http://community.o2.co.uk/t5/Pay-Monthly-and-Pay-Go/iPhone-6-cost/td-p/858113 but further searches will confirm

I can confirm that.
I got the iPhone 6 64Gb two years ago on refresh. The phone at launch direct from Apple was £619.
I was was charged by O2 £249.99 upfront and 24 X £20 which is a total of £729.99, £110 more than direct from Apple

Iphone 7 Plus 256GB from O2 = £180 + (24x£35) = £1020
Iphone 7 Plus 256GB from Apple Upgrade = £50 + (20x£50) = £1050
You do get AppleCare+ thrown in with the Apple Upgrade, but the point is it's not just O2. You're paying for the priveledge to spread it across 24 installments. Don't other places like car insurance charge you a fee to pay in installments?

@Anonymous
Did you read the 0% interest rate, that is no charge. so doesnt account for the extra.
I just guess its because the o2 parent company are £40bn in debt.
