I think the best thing to do now is to call customer services http://www.o2.co.uk/contactus as it looks like everything has been "improved" again.
I have no affiliation whatsoever with O2 or any subsidiary companies. Comments posted are entirely of my own opinion. This is not Customer Service so we are unable to help with account specific issues.

Currently using:
Pixel 7a (O2 & Lyca), One Plus 6 (Sfr), iPhone 12 Pro Max (Vodafone)

We've got the confirmation that the Final Bill Refund Form is no longer available. They are already working on a new system to improve the quality of this service. 

However, while moving to the new system, the faster way to get your refund is to get in touch with Customer ServiceThey would be happy to help you with the refund! (Note: Anything over £20 will get auto refunded)
