Cannot unlock iPhone

I have an iphone 4, I've tried to unlock it using the o2 unlock form.
It's sent a message back today saying that the phone could not be unlocked.
"For more information, go to this website... www.o2.co.uk/unlockmyiphone"
Unfortunately this website they gave me offers no explanation and is complete rubbish.
Can anyone give me an idea of why it can't/won't be unlocked?
My balance at the moment is £19

so you have an iphone 4 16 or 32gb on payg and want to unlock it?
1 is the phone actulay locked to o2?
2 you have recently used it with the sim to make calls
3 is this a new phone to you i.e where did you buy it.
the more info you give we may point you in the correct direction.

It's the 16gb version.

I bought it off ebay and was told it's locked to o2 uk. o2 Sim card works fine in it. (I'm afraid to cut up my other sim card incase it doesnt work in the phone)
I topped up by £20
I've used the sim to make 4 calls....(I was told I only needed to make 2 calls to a UK number)

No, I am in Ireland.
Should it still work?
I imagined it would not make a difference seeing as i topped up and made a few calls to the uk.
And i can receive messages from the uk.

It won't work if you are in the South if you have a UK sim and the phone is locked to O2-UK. Not sure on O2-IE's unlocking policy but if it's a UK sim, you have to be in the North or on the UK mainland.

As Stevyjones says the handset needs to be in the o2-uk coverage area when the calls are made to register the sim to the handset in the uk and request the unlock.
