Hello,

I was previously on a month to month contract but now I would like to do a 12 month unlimited plan with the same SIM card, is that possible?

Yes, just upgrade in your MyO2.

Dont forget it is possible to get O2 deals via other companies and still maintain the same SIM.  For example I moved to a £6 / 4GB SIM only deal via MSE (was not available direct from O2 shop) and it was still treated as an upgrade so keeping the same SIM.   -  The MSE link simply took me to O2 basket with the £6  /  4GB   pre-added - and applied there.  Never assume your only upgrade option is direct to O2.
