I top up my mum’s PAYG for cost of the tariff ready for the 16th. I did it as I always do, possibly a little earlier than normal, and it was acknowledged as normal. However it’s applied to her overall balance and she received a text this morning ‘You need enough balance on 16 April 2020 to cover the cost of your tariff next month.’ reminding her to top up before the 16/04.

Is there just a lag in the system updating? Any ideas?

Yes, currently there are all sorts of issues with O2's systems, which aren't being repaired due to the lockdown.

If you had an acknowledgement of the payment, all should be fine.

Just check what happens on the 16th.
