I don't think your watch needs a QR code when changing phones, but you will need a new one for the phone.

  1. Erase your Apple Watch.
  2. Set up your new iPhone and sign in to iCloud. ...
  3. Open the Apple Watch app on your new iPhone, then pair your watch with your new iPhone.
  4. Try to restore from a backup. ...
  5. Follow the onscreen steps to finish setting up.
  6. Start using your Apple Watch with your new iPhone.

You should have said wink

A new QR code is only needed if changing the device it's used on.

You can reuse the same QR code on the same device up to 3 times.
