Does that mean o2 still test the phones on the firmware and update the oems to say it ok or do the updates come straight from the manufacturers

I presume they still come from O2 unless it's an iPhone.

I’d thought that if you bought the phone from O2 then that would come pre installed with O2 settings until you put a non o2 sim in

It will depend on the regional sales code of the device as to where the updates come from.
