Hi I recently lost my phone and rang o2 to bar my phone and sim, I have received the new sim but my insurance company is asking for proof of blacklisting. How do I go about getting proof?

To add to @madasaf1sh's advice above, Guide: Lost or Stolen O2 Device? - How to Report it contains the info you need to make sure your phone is properly barred - and explain that your insurer, similar to this one shown below, needs info as laid out below:
