The form to get the PUK code doesn't work, I am unable to talk with Support and I am locked out from using my Sim card. Please, help.

It's also available in your MyO2 as well as here https://www.o2.co.uk/help/device-and-sim-support/unblock-your-phone

Thanks all! I was finally able to find it from Myo2 menu. It’s been a very frustrating experience tho.
