Phone returned for repair

I sent my phone back for repair yesterday, but when i got home and read the paperwork, it said i should include page 3 of the paperwork. I have sent the phone only and no paperwork, as originally reading the first page of the paperwork it says to send the phone only
I am worried now that the phone will either be lost or any repairs delayed
I have been sat on a phone in a Que for over 45 minutes to try and speak to someone but it's absolutely hopeless
Can anyone here help or advise?

The repair centre may send it back, they might repair it, but you need to keep on trying o2 and speaking to someone, but I doubt anyone in the call centre can do anything.
Is there a contact number for the repair team on the paperwork, as it might be quicker.
There is also no o2 support on this customer to customer forum..
