Hi, 

I've been using an iPhone 6s on 4G but am now waiting delivery of an iPhone 12 on a 5G package. Am I correct in thinking my current sim should work on the iPhone 12 connecting to 4G service and I only need to swap sim to connect to 5G. Thanks in advance.

Yes, correct.

You should get a new sim with your new phone so you just need to do a sim swap Guide: Sim Swap: a mini guide 2017 update
