Baffled by setting up new phone, need a QR code

I'm trying to set up a new iPhone and am going round in circles trying to either request an 'eSIM' or find a QR code provide by O2. The old phone won't pair with the new one no matter what I do, so I can follow thw prompts to continue. But I'm happy to enter the info manually. However, I don't know what in input, or how to get the QR code. Where does the QR code come from??

eSIM - How to activate  is the current process for setting up an esim based contract. The esim used to come as a box with a QR code on it, O2's process has been improved.

Or take phone and head into an O2 Store https://stores.o2.co.uk/ ,  with photo id, and let them do the upgrade there - switching back to a physical SIM is probably a good idea, if your handset supports it, @BaffledbyOtwo.
