02 putting a default on my credit report

I have a default on my credit report from o2 even though I have never missed a single payment since I became an 02 customer over 5 years ago how do I get this removed ?

You may also be just as well raising a dispute via the Credit Reference Agency as well, this can be done via one of the Credit Score Sites or direct
