Hi @Anonymous! You can find info and the correct forms for changing the Name on your account here.
The text currently says it's for Business customers but the same forms can be used for personal accounts too. There is a piece of work going on to change the language on the forms to avoid confusion but you can use them all the same for now. Any problems or questions please ask!

Hi @Anonymous Please be aware that the link to the form shown above just sends you in a loop by to customer services.
It has been reported and still not fixed.
In the interim you can send scanned copies of the documents required (with a covering letter) by post to

As the online form is no longer working we are told you now need to go in store to get a form.
Take your marriage certificate or deed poll with you and complete the form in store and they will fax it all off for you.
Note name changes can only be done if it's the same person changing name and not into a different person.
