Resolved! Voicemail and pin being reset by system

Over the last couple of years, I randomly discover that my outgoing message and pin have been reset to default and I have to do it all again. Is there some O2 policy causing this to happen? Found other questions like this - but with no proper answer....

I recently tried to pay a bill on o2 website using my debit card and then there was something wrong with the payment on your site so i was charged and i tried again and the bill went through ok with the same debit card i was promised a refund of 76.9...
