Hi,
Does anyone know what happens if a contract phone goes faulty after the first 12 months?

All the info you need is on this page
http://www.apple.com/uk/legal/statutory-warranty/
Apples 12 month warranty is in addition to what is required by english law / eu regs
If bought through o2, o2 is the retailer and the one who you contact normally but apple offer the enhanced warranty for the first 12 month so contact them.
Then you have to prove to o2 that it is a fault that was present at point of delivery after the apple warranty has expired.
In the uk no law exists saying the manufacturer has to provide a warranty only the retailer under the sale of goods act For info on that see this usefull which help page. http://www.which.co.uk/consumer-rights/regulation/sale-of-goods-act/

Depends what phone you have, most come with a 2 year warranty from the manufacturer.

Most are covered for two years but for others you may have to make a claim after 1 year, and there is still much argument about this:

Hi,
I'm going to get an iPhone 5s and just wondered what was covered in the 24 month contract.

Some home insurances or bank accounts give cover too.
Which wouldn't cover you for a faulty phone, only damage or loss.
