Edited to add - you normally have 30 days to use your PAC number, so unless you have held on to it you should have some time to get it resolved.

Are you sure you have a PAC and not a PUK ?
A PAC should start with 2 or 3 letters.
It's deffo the PAC, it starts with 3 numbers, I've confirmed it with my old provider twice today.
I've filled in the form linked above, will see what happens over the next couple of days, it dosen't work I've got the 14 days to change my mind about coming to O2
