How do you know you have reached your limit on Downloads etc. and if O2 are or will be then taking more payments from you than your agreed contract bill.

Is this why I hear some peoples monthly bills are outrageously expensive?

O2 do not charge for going over your data on a monthly contract. They just slow your connection until next month, so you won't get a huge bill from data use in the UK.
