PAC code help

I have today bought a new phone contract for myself, and a sim only deal for my partner (under my name). We have moved to o2 frpm Three. At the time of purchase we didn't have PAC codes as it wasn't working so we were given 2 sims with 2 different numbers obviously. The lady explained once we got our PAC codes we switch our numbers to keep the old ones. This evening managed to get out PAC codes through in a text. I log into my o2 account and click on the option 'keep, cancel or take on a number'. We do this for my partners number first, and it worked. However, we then tried my number but it won't allow us, doesn't give us an option. I tried filling in the form but again it won't recognise the o2 number or SSN. Can anyone advise how we change my old number across to o2 aswell please? As I keep getting the 2nd image come up on the screen even though that's my partners number ans PAC code.
