Mysterious regular payment

I’m not very good with keeping on top of my bank account activity but I have noticed that i’m paying 4 direct debits to O2 but I only have 3 contracts.
Any obvious reason for this before I try to contact them direct?

If one is a refresh contract you make 2 payments.
One for the phone and one for the airtime.

Having been the victim of bank fraud twice, I check my online banks every couple of days. The only way to ensure everything is as it should be.
