It is a long time for the swap to take place. You need to keep rebooting your phone (turning it off and back on again) If it doesnt work then do it again.. http://swapmysim.o2.co.uk/
If after all that it still hasnt worked then ring customer Services..

Do it again or pop into your O2 store.
You won't mess things up as it hasn't been actioned.

Hi there @Anonymous
If you are changing from pay and go to contract, then you need to follow the instructions for keeping your number. The form you have sent doesn't work in this case.
this is the correct one http://www.o2.co.uk/help/phones-and-devices/how-to-keep-your-mobile-number
Hope this helps.
