Can someone explain why I entered a 12 month agreement for a fixed price back in December and since then you've increased my bill twice? Surely this is a breach of the contract which was agreed for 12 months...

As per your T&C's which you agreed to in December it quite clearly states that there are price rises every year in April of 3.9%+RPI
So no breach of contract.

There was the annual price rise in April this year which was written into the terms and conditions to which you agreed.
There won't be another till next year so not sure about 2
