Try contacting O2 for your account details which will enable you ti initiate the cancellation process.
message them on Social Media if that works best for you -on Facebook (https://o2uk.co/O2CFB) , Twitter (https://o2uk.co/O2CTW) or Instagram (https://o2uk.co/O2CIG)
You can also try going in store with suitable forms of photo ID and see if they can find your account details.
