I write to give 1 month notice to disconnect the captioned line, effective today's date (10/8/2014) because I moved to Hong Kong.  Please be noted that my address has been changed

to : xxxx

 

my new Contact No. has been changed

 

Thank you for your kind attention and assistance to disconnect the captioned line.

I know I've said in the past that people can cancel by any method of their choosing as long as they prove it has been received, but I'm not sure this counts
