This may not be what has happened, but this could be the result of someone hacking into her account and ordering a new phone and contract.
To check this go into her MyO2 account and check all is normal and no orders have been placed without your knowledge.
If there are, ring O2 and ask for the fraud department and get them to investigate, also inform the police and change the account password.
You can ring them on 202 from a contract mobile free.
