Activation date & monthly pay date for purchasing a monthly plan SIM

I am using a O2 PAYG SIM and I am considering to change to a Monthly Pay SIM.

If I ordered the monthly pay sim online,  it will be delivered to my home (by Royal Mail), and I have a few questions:

1. Is it the order date being the day of month for each ongoing monthly payment?

2. When will be the monthly pay SIM being activated? Is it the first time I plug it into my mobile?

3. My O2 PAYG account has some credit (around £2 to £3), when I apply the change of number of the monthly pay SIM to my original number of PAYG, will be the credit gone?

Payment starts from the order date and is due 14 days after that 

The PAYG balance will be transferred
