My sister has a pac code from Sky Mobile and it seems finding the online form was a knightmare but she did find it filled it in to transfer her number from Sky Mobile to o2 pay and go big bundles package and after a few mins got a screen saying your transfer is under way then after 10mins text message saying o2: Sorry we are unable to process your port in request please call us on 03448090202.
She has setup amd account on the My o2 wedsite and downloaded the My o2 app to her iPhone 16e she also fined in here right name but the My o2 app is for some reason shown her name as Hi null.
She also been on live web chat and they was not much help and told her to phone customer service.

Contact O2's social media team using either FB or Instagram as per the links in here, @darrenney02 - Guide: How to find help & contact O2 - they are usually better equipped to handle such technical issues between MyO2, PAC and number transfers from other providers into O2. Good luck!
