In 1994 a Visa credit card of mine was relieved of £3900 possibly due to an internal issue at the card issuers.
It was replaced immediately of course and the replacement card is still in use and has had no problem.
Certainly nothing to do with keylogging though.
I've also had my O2 account hacked and a new phone and contract set up without my knowledge, fortunately I was onto it before O2 realised what had happened and was able to reverse it within hours.
This was unfortunately due to a lack of security at an O2 call centre.
