This isn't O2 you are talking to, it's a community forum.

Proof of purchase could be a copy of your contract or emails sent if done online.
Otherwise if you call customer service they can provide it for you http://www.o2.co.uk/contactus

Another method of proof is your orders which will be showing in your MyO2 http://www.o2.co.uk/myo2
