Printable sticker for insurance return

I was told when returning my broken phone to o2 to claim on my insurance i would be sent a printable sticker to put on the envelope containing my faulty handset, however neither of the e-mails i have received contain this sticker or even a link to it. Any help would be greatly appreciated.
