Generally happy with the service.
The following things desperately need looking into:
Free calls to the office (dedicated landline number(s))
Signal quality - desperately poor in our area and across the whole estate.
If the signal quality doesn't improve significantly, we'll be moving to Vodafone when the contract expires. (Do you have any of those Sure Signal things?)

Number 1 near real time usage information like consumers tariffs as new tariffs come with data caps it would be good to use my-o2 iphone app etc for small businesses like ours call usage monitoring not so important. EDIT just seen new banner in myo2businessaccount looks like this is on its way!!
Addressing business queries on the forums.
As previously stated femtocell availability (not just for 3g but as a fallback in the event of local cell failure (business continuity))
I am sure I can come up with a long list.

For the love of sweet baby Jesus SIGNAL QUALITY.
This is ALL that matters.
Why don't O2 get on the clue train and ATLEAST offer Femtocells.
I'd be an O2 customer for life if I could just drop £100 and get a Femtocell to fix the absolutely horrendous signal quality in Lower Holloway / Islington / Barnsbury where my business is.
It's ridiculous how many times I've rung up to try and give feedback on this issue and they outright say "Yes, - we can't really do anything".....
Waiting for contracts to expire.... will be moving on.

The biggest issue all round is the signal strength... It's incredibly poor.

When you sign in to mybusiness account to view bills etc there is a banner on the right that allows you to change them.

In desperate need of 3G signal pretty pretty pleeeeeeeese in po368bn which is sandown isle of wight i think networks dont realise we are here I know alot of ppl that are u2 business and consumer myself vein business user and Jst Gavin GPRS is no good also an iPhone app like the consumer one would be grate apart from everything is good

3G rollout in rural areas would help rural businesses in adsl slow/not spots
Three are already here!! o2 3g 5 miles away yet I can see the 2g o2 mast from the office window 2 miles away.
I know I can get a 3 3g signal looking at my iphone carrier info if I enable 3g

Signal Quality, Signal Quality, Signal Quality.
Easier access to billing details. I like the simple email with total amount, but a link without having to reset my password each time as I can't remember it, or the full bill as a pdf attachment would be SO MUCH EASIER..
And some kind of tie-in with East Coast rail to get decent 3G coverage all the way up the line between London and Newcastle would be good, especially as their wifi has always sucked and now has to be paid for...
Oh, and cheaper calls/contracts would help us all in a gloomy financial climate, of course...
