No Insurance Return Instructions

My phone is getting collected tomorrow (16/04/2020) and being replaced due to a broken screen. When I spoke to O2 Insurance earlier they said I would receive an email with instructions of how to package my device. I've still not received this email and have no idea what information I need to send when the courier collected it.

Can anyone help me with what information I need to include/write on the packaging?

Just put some bubble wrap around it.

The courier will open it anyway to inspect and photograph it and they will have an envelope to put in in for returns.
