How does this work? Do you have to phone a number? Is access given immediately?

All this information should be contained on the leaflet you get with the Micro SIM from O2.

If it's the same as Mobile Broadband, if you try to connect to the network it should take you to the top up page:

Put the sim in the iPad, sync with iTunes, check for updates, accept the update, select view account and enter your details to register, you'll then be prompted to select a package.
Do everything on the iPad. Don't top up anywhere else as that will be money wasted.
iPad prioritises wifi so don't bother registering an account until you need to ie have no wifi coverage.
