First and last time using SIM card and IMEI number

I am trying to make an isnurance damage claim on my phone but cant get through to customer services to get the records for the first and last time I used my SIM card.

I also need proof of purchase and my IMEI number. There is no email to write to and I work from work so need my phone.

Can anyone please help?

If you log into MyO2 via the website you will be able to get all of that information and documentation that you require.
