I recently used the online chat to upgraded a Lumia 520 to a Lumia 550. I understood from the conversation that the new phone would be delivered with a NANO SIM alrready fitted. It wasn't. What's the advice on resolving this please?

If you go into an O2 store as @Cleoriff suggests, they will give you a nano sim and also swap your account details from your old sim to the new one. It can take up to 24 hours for the sim swap to take effect, but it's usually much quicker than that. I did it a couple of years ago and the sim swap process took about 10 minutes, but times can vary.
