I have kidney problems and I can tell you, the doctors have to be very careful what they prescribe.
Some info here as to why your doctor may not feel prolonged use of painkillers could cause you problems

Before taking tramadol
Some medicines are not suitable for people with certain conditions, and sometimes a medicine may only be used if extra care is taken. For these reasons, before you start taking tramadol, it is important that your doctor knows:

I can confirm it is NOT a drug recommended for long term use.
As I said... whilst I understand the need for changing your drug regime, I can't understand why you haven't been referred for further tests.....
*Pain is what the patient says it is* is something medical staff should know and adhere to.
However they have a duty of care to discover WHY you are still in so much pain...

Oh don't even get me started on the issue of a GP.
I had so much trouble with them it is unbelievable.
Painkillers are supposed to be on repeat (long term hip problem) and the doctor randomly decides to stop it without telling me so I have to ring at 8:15am every morning to try to get an appointment with the GP to talk to him about it as he won't discuss it over the phone. On hold for over 30 minutes every morning and when I finally get through they say there are no appointments left.
Then when I finally see the doctor (locum doctor) he tries to give me different medication which I am allergic to. I ask for the same medication that I have had for many years. He doesn't even look at my face nor does he examine me. The whole time he is staring at the computer.
My advice is to not suffer in pain. Some GP's don't understand and aren't compassionate. If you pain is bad always go to the Walk In Centre or A+E. They can't turn you away @Anonymous
I hope you sort it out soon!!

A GP is absolutely right to try alternative drugs to ensure you are not becoming immune to those you are taking...or even worse case scenario...becoming addicted.
I've been taking codiene every day for the last 18 years.......im concerned now that i might suddenly become immune as no other medication has helped
