Sending Phone for Repair

My phone has been booked in for repair and I have received some paperwork to return my phone via the post office. However, there is no address to return it to - I just have a repair assessment form to send back with it no address label. Can anyone help please?

There is a returns form and address here https://static-www.o2.co.uk/sites/default/files/2017-05/mobile_device_return_form.pdf 

But I would check with Customer services forst if it is current Guide: Coronavirus Community Help and Support

Hi @Elliebir welcome to the o2 community forum.
First and foremost before sending your phone anywhere take a photo all round. Note the IMEI number so you know your phone is returned.
Only send TRACKED insured for loss.
Who is carrying out the repair?
Are we assuming it's o2.

I have a repair assessment form from O2 so yes I'm assuming them and they asked for my IMEI number when I booked the repair

It would be necessary to contact customer services. You would need to be patient because of low staffing levels due to covid-19.
However better completely safe and not sorry.
