O2 Recycle Pack

I’ve recently upgraded my phone and was supposed to receive an O2 recycle pack to return my old phone and receive a credit. I haven’t received the pack but have had an email that I have 14 days to return the phone. How do I return the phone without the O2 recycle pack?
