You partner will need to take out a new contract in their own name and pass the Credit and Affordability Checks, once they have there own contract you can request the current number to be migrated to the new contract

Also note before you can migrate, you need to pay off the existing device plan / contract

If your phone is no longer in your possession, you could report it to O2 as lost or stolen - O2 would cut off service (but you have no guarantee your partner would return the phone to you ...). Just a thought.
