New sim query

hi, I've purchased a new p&g o2 sim for my hubbys old 02 Galaxy s4. It has a phone number on the card so following the enclosed instructions I popped it in his phone and it still said no sim. I tried to top up and sign into my o2 to send an activation code but neither were successful or had any effect. 

Any ideas?

The packaging states it's still in date but clearly something's wrong.....
