if the questionable number she gave you was 202, then that is the shortcode through to pay monthly customer services from your phone. She should have given you 4445 for pay and go.
I have had a few phones unlocked through customer services and the online form. Sounds like you have just been really unlucky.

It will get worse before it get's any better.
I have never had a problem using the standard form and I've unlocked a fair few phoness over the years.

I to am having trouble with O2 unlocking my iphone, since ive requested the PAC code.
They quoted 72 hrs to unlocked it, and i set my new provider to to the pac code when the 72 hours was up.
O2 didnt unlock my iphone after 72 hours, and blamed a "technical" fault
Now im left with no working phone!
O2 are now quoting ANOTHER 72 hours to unlock the phone, thanks O2. Ive been with O2 for many years, and they were not at all interested in compensation or leaving me with no working phone.
Get you phone unlocked well in advance of leaving O2
I will update what happens next.

So am I, this must have been a terrible hurry they were in.
This mistake may well slow them down a bit.

when you are told "72 hours to unlock your phone, and usually its much quicker" you dont think theres a problem to give the PAC code to another provider, to be active after the 72 hours.
