You actually need a data only sim for it to work in any tablet.
The pay monthly T&C exclued the use of sims in modems Tablets are not mobile phones
Your tariff usage is subject to our 'excessive usage policy' (outlined in this clause 16 and clause 17 of these Tariff Terms) at alltimes. This means that your usage must be for private and personal purposes and that of a legitimate consumer's. You may not use your SIM Card:

Hi,
No they are different sizes.
http://support.apple.com/kb/HT5554
Your iPhone 4s takes a micro sim.
The ipad air takes takes a nano sim.
Here is the link to ipad sims http://www.o2.co.uk/broadband/ipad-sim and the tariffs for iPads.

Technically possible although it would be easier to set a. Personal hotspot on your phone and tether your iPad to it.

As long as the OP is on a Pay Monthly Contract and not on Payg then thats certainly possible for Tethering from the iPhones inclusive data to the iPad.
Although its also worth mentioning that the data allowance would drain more quickly.

Only o2 can answer this but it has been discussed before from memory.
If it is sold as a tablet then o2 see it as a Tablet and not a mobile phone and it will fall foul of the T&C
