I am trying to unlock my I Phone and i have filled in all the fields on the form provided. My phone is also registered with O2 but i keep getting this e mail back

Have you used the simcard in the iphone for calls and texts. Last time I unlocked an iphone the security question related to the numbers most called/texted.
